1.  2 references.  One from the veterinarian that will be caring for the puppy and one from a neighbor, friend or clergy.

2.  1-2 paragraphs about your family and home life. Do you have experience with Old English Sheepdogs in the past? 

Please explain. Do you have a fenced yard or invisible fencing? Give me an idea of the average daily schedule around your home, ages of any children or grandchildren, nieces, nephews ect that will be in frequent contact with the puppy, and any hobbies that will involve the puppy.  Will the puppy travel with you on vacations or be boarded (I  do not have a problem with boarding kennels), how many hours will puppy spend alone during the day, how do you plan on socializing the puppy, what activities are you planning on doing with the puppy, and anything else you would like to share.

3.  I use this information to help place the right puppy into its forever home.  If you are happy, then my puppy will be happy.

4.  If a puppy is available, I require a $500.00 non-refundable deposit within 7 business days (check, money order, or cashiers check is fine).  Balance to be paid in cash when you pick up your new family member.
